SCIENTIFIC BACKGROUND

Diabetes Mellitus has been described in ancient ayurvedic texts under the name ‘Prameha’. They have also explained in-detail the etiology for the disease and various herbal remedies for the same. “Comparative evaluation of efficacy of use of Munident and toothbrush with toothpaste on salivary streptococcus mutans count and gingival health of children- an invivo study ”

ABSTRACT :

Aim – Dental caries is a multifactorial disease which has a deleterious effect on the oral cavity . Improper oral hygiene habits is a cause for the same . The aim of this study was to compare the antibacterial efficacy of Munident , an ayurvedic dentrifice with commercially available toothpaste .
Material and method-  A total of 40 students of the age group 9-12 years who were resident of Bala Yeshu Nilaya Bhavan , Mangalore were chosen .They were divided into two groups .Group 1 (control group ) and group 2 ( study group) . The DMFT scores noted and divided into two groups consisting of 20 students each . Group I was instructed to brush using toothpaste and toothbrush and group II was instructed to brush using munident and toothbrush and their gingival bleeding index was noted pre and post rinse . Also salivary streptococcus mutans count was noted pre and post rinse .The results obtained were subjected to statistical analysis .

Results – the herbal dentrifice showed better efficacy in comparison to toothpaste
Conclusion – Usage of herbal dentrifice has similar antibacterial effect as conventional toothpaste . Hence the practice of using dentrifice with natural ingredients should be encouraged.

Mechanical plaque removal is one of the most accepted methods of controlling plaque and gingivitis. But it is expected that less than one-third of the population can effectively perform mechanical plaque removal. Several chemical preventive agents have beneficial effects in the control of plaque and to reduce or prevent oral disease. Hence, various chemical formulations were tried in dentifrices. Chemicals, mainly triclosan and chlorhexidine, have been added in mouth rinses and dentifrices to prevent plaque and gingivitis.

Our aim of work – To evaluate the use a toothpowder, Munident made  of purely natural ingredients which does not contain any chemical compounds or artificial colours, sweetners , preservatives that harm our tooth as well as buccal cavity.

MATERIAL AND METHOD

The study was initiated after obtaining  consent from the orphanage and ethical clearance from the institute (IEC/Rev/25/2015-2016) .40 healthy children in the age group of 9- 12 years were selected from Bala Yeshu Nilaya orphanage of Mangalore city .The study and treatment protocol was explained in detail to the parents and the child and informed consent was obtained for the same .

INCLUSION CRITERIA:

1.  Systemically healthy subjects.
2. Atleast 3 restored/decayed and/or missing teeth(dmft/DMFT)

EXCLUSION CRITERIA :

1. Children who cannot expectorate completely.
2. Children who could not brush their teeth on their own.
3. Children with a history of taking antibiotics 3 months prior to and during the study period.
4. Children undergoing orthodontic treatment or with an intraoral prosthesis.
5. Presence of any intra oral pathology.
6. Medically compromised children.

They  were selected based on the inclusion criteria and divided into two groups .The gingival bleeding index was noted prior to collection of saliva using periodontal probe .Each tooth was divided into four parts – Disto facial papilla, Midfacial papilla , Mesiofacial papilla , entire lingual gingival margin and scored according to Gingival index ( Loe and Silness, 1967 ).

SIMPLIFIED ORAL HYGIENE INDEX:

Simplified(OHI-S index) Index was described by John C Greene and Jack R Vermillion in 1964.
Instruments: mouth mirror and explorer.
Teeth selected:16,11,26,36,31,46.
Surface examined:
4 posterior and 2 anterior teeth,buccal surface of selected upper molar and lingual surface of selected lower molar.

Criteria for classifying debris:
Score 0-  no debris or stains.
Score 1-  soft debris covering not more than 1/3rd of the tooth surface, or  presence of extrinsic stains without other debris regardless of surface area covered.
Score 2- soft debris covering more than 1/3rd , but not more than 2/3rd of exposed tooth surface.
Score 3- soft debris covering more than 2/3rd of exposed tooth surface.
Criteria for classifying calculus.
Score 0-  no calculus present.
Score 1- supragingival calculus covering not more than 1/3rd of the exposed tooth surface.
Score 2- supragingival calculus covering more than 1/3rd but not more than 2/3rd of the exposed tooth surface or the presence of individual flecks of subgingival calculus around the cervical portion of the tooth or both
Score 3- supragingival calculus covering more than 2/3rd of the exposed tooth surface or the continuous heavy band of subgingival calculus around the cervical portion of the tooth or both.
OHI-S  score for each individual is calculated by,the debris and calculus scores are totaled and divided by the nimber of tooth surfaced scored.
Average of each individual debris and calculus i.e simplified debris index (DI-S) and simplified calculus index(CI-S) will be scored from range of 0-3.
Good- 0.0 – 0.6
Fair- 0.7-1.8
Poor- 1.9-3.0
Sum of the DI-S and CI-S will give the OHI-S values ranging from 0-6.
Good- 0.0 – 1.2
Fair- 1.3-3.0
Poor- 3.1-6.0

The children in the first group were asked to brush dialy using toothbrush and toothpaste which was provided to them . The children in the second group was asked to brush with munident which is available in the form of a tablet . The children were instructed to crush the tablet and add two drops of water and use it with tooth brush for a month .

A base-line non-stimulated whole salivary sample (2 ml) was collected in the morning    by  -asking the patient to drool passively into a sterile plastic bottle for 5 minutes. Subjects were informed in advance not to eat or drink (except water) one hour before saliva collection to minimize possible food debris and stimulation of saliva. The samples collected in sterile bottles will be carried in the ice box containing ice (used as transport media).The. Samples were tested for the number of colony forming units for Streptococcus Mutans using Mitis Salivarius Bacitracin agar.

Laboratory procedure

The sample was vortexed to uniformly mix the saliva and media using a Cyclo mixer. Using an inoculation loop (standard loop with 4mm diameter) 10?L of the vortexed sample was streaked in duplicate on Mitis Salivarius Bacitracin agar selective for Streptococcus mutans. Themitis salivarius agar plates were incubated in aerobic conditions for 48hrs at 37 degree Celsius in an incubator.The plates were opened after 48 hrs. The counts were made of colonies with morphologic characteristics ofstreptococcus mutans (0.5 mm raised convex undulated.

colonies of light blue color with rough margins, granularfrosted glass appearance)4 on the plates using a magnifying lens and were expressed as number of colony forming units (CFU) per/ml of saliva (Figure 1). Semiquantitation of the number of colonies was done by multiplying the actual colony count with 1×103 as the sample was diluted one thousand times (1:5 dilution). The Baseline scores were noted.

The statistical analysis used was paired t test to assess the gingival bleeding index and streptococcus mutans count was assessed using Mann Whitney U test .
RESULTS- A total of 40 children participated in the study . At baseline Group 1 had had a mean Streptococcus mutans count of 73800 counts/ml and group II  75608counts /ml. When Man whitney U test was performed, the p value was not significant. Hence it can be inferred that the statistic difference was not significant. (Table 1)
The gingival bleeding index in group 1  pre rinse was  1.8050 using toothpaste and using munident was 1.5450, p value (0.008) has significance. (Table 2 ) The gingival bleeding index post rinse  was 1.6700 using toothpaste and 1.4300 using munident , p value (0.009) has significance .

DISCUSSION :

Dental caries being a multifactorial disease process requires a multimodal approach to treatment and control. The primary purpose of this study was to evaluate the efficacy of herbal dentrifice in control of plaque and gingivitis . The study compared a herbal toothpowder Munident with a standard control. Both Munident and toothpaste were found to be useful in effectively reducing the Streptococcus Mutans  count  and gingival bleeding index score after usage for 30 days.
By comparing the change score between the two groups, it can be said that the difference in score of Munident  group was slightly more than that in positive control group, but the difference between the two groups was not statistically significant (P>0.05) and thus superiority cannot be claimed. However, it can be said that Munident  was as effective as that of positive control group in reducing score of Streptococcus mutans and gingival index. The strength of the study was its randomized controlled design.

The drawback of the study was that it was done in relative small sample size and thus it was not powered to show a statistically significant difference between the two groups. Using natural medicines to cure various diseases has become an increasing trend. Herbal medicine has made significant contribution to modern medical practice . the practice of using natural products should be widened  to reduce the side effects and to ensure a good oral hygiene .
CONCLUSION :
India being a country where use of  traditional medicine is accepted widely  . The practice of using dentrifice like Munident which is made of natural ingredients should be encouraged  keeping the potential benefits in mind .Publicity of its use using appropriate media would benefit the general population by giving more confidence in the ancient practices , thus preventing tooth decay and loss.
